Job Title: Interim Housing Case Manager

Reports To: Program Manager
The Alcott Center for Mental Health Services, a nonprofit community mental health agency, is seeking an organized, motivated, and professional candidate for a Full-Time position as a Case Manager for our Interim Housing Site. The on-site Case Manager plays a pivotal role on client’s treatment team. Having the most direct contact with clients on a weekly basis allows for specific insight into a client’s progress and challenges.

Essential duties and responsibilities include, but are not limited to the following:Intake:
- Meet with new clients within 24 hours of entering the program for an initial meet and greet, intake tour, and orientation to the facility and rules.
- During the first engagement focuses on creating a welcoming and supportive environment. This includes providing trauma-informed care, and harm reduction philosophy. Rapport building is key during this first engagement.
- Completes all intake documents within the first 72 hours. Documents completed after the 72-hour window require approval from the Program Manager.
- Thoroughly review intake documents including client rules, expectations, and policies, and gather signatures for all required documents.
- Follow up with on-site medical staff to ensure that clients' prescribed medications have arrived
- Select room assignments and coordinate with Resident Aid Staff.
Client Care:
- Schedule weekly check-ins with each client on the assigned caseload. After each check-in, enter notes into Champ that details the content of this meeting. All client notes must be submitted before the end of each calendar week.
- Utilize the client's needs checklist and obtain needed documents such as ID, Birth Certificate, Social Security Card, income benefits, etc.
- Assess clients for active substance use, changes in mental health presentation, medication compliance, and overall engagement with programming.
- Take the lead in advocating for clients' needs and scheduling appointments. This includes transportation to appointments.
- Review the end-of-shift reports provided by staff to track client progress and overall compliance with guidelines.
- Investigate all reported concerns detailed in the end-of-shift reports.
- Check emails regularly, as well as clients' CHAMP profiles, and Diversion Database notes.
- Attend team meetings to support staff and clients' needs.
Treatment Planning and Documentation:
- Communicate with the Program Manager regarding the overall progress of assigned clients during weekly meetings.
- Continue the upkeep of files including updated signatures, consent forms, and documents (ID, Birth Certificate, Social Security Card, etc.
- Complete 5x5 assessments, and care plans for each client.
Qualifications:
- Experience working with clients with mental illness, chronic health issues, and substance use disorders.
- Knowledge and expertise in the following areas: chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, and housing.
- Able to perform basic computer skills required for an electronic health record.
- Evidence of clear criminal record and proof of valid California Driver’s License
- Must be fully vaccinated for COVID-19, including booster.
